To obtain a death certificate you must be the parent, child or spouse of deceased.  You must have a valid picture ID.  If mailing in, print application and mail in with picture ID and a $10 money order per copy.  If you are an attorney, you must have a legal need for certificate. 

 

Application to Local Registrar for Death Record (DOH-294A)

If applying in person come to the first floor of City Hall with some proof.

 

If person passed away at St. Lukes Hostiptal after May of 1957 apply to the New Hartford Clerk at (315) 733-7508 ext. 2325.
